How to Submit a FOIA Request

FOIA requests to the NNSA/Office of the General Counsel may be submitted online electronically, by e-mail, fax, or in writing.

To file a FOIA request, please include the following information:

• Name (Required)
• Full SSN of individual whose records are being sought
• Organization
• Death certificate (For 3rd party requests)
• Address (Required)
• Phone Number (Required)
• Fax
• E-mail Address
• Reasonably describe requested documents (Required)
• Requester Identification (i.e. individual, educational or noncommercial scientific institution, private company, or media) (Required)
• Your agreement to pay fees or request a fee waiver (Required)
• If required, request expedited processing (example: an imminent threat to the life or physical safety of an individual exists or an urgency to inform the public concerning actual or alleged Federal Government activity exists)
